Attention all Kingdom Sports Staff members...you can now download our TIME OFF REQUEST FORM, SHIFT COVERAGE FORM, AND A TWO-WEEK NOTICE FORM. It is your responsiblity to communicate with your supervisor and to cover assigned shifts. Use these forms and the assigned directions of responsiblity to cover shifts and request time off. If you have questions please contact your immediate supervisor at (812) 962-3478.

Make sure that you are checking your e-mail and communicating with your supervisor on a consistent basis. You can call the office at (812) 962-3478 or e-mail your supervisor at any time.

NOTE: Your supervisor is typically in the office between 10 a.m. and 2 p.m. At 2 p.m. they typically head out to the gym for training and programs. Thanks and have a blessed day.
